Upon release, The Doomstar Requiem polarized audiences. Hardcore Metalocalypse fans expecting non-stop gore and gags were jarred by the slow pacing and earnest musical numbers. However, music critics and metal journalists praised it as a groundbreaking fusion of genres. The A.V. Club gave it a B+, calling it "a bizarre, beautiful, overstuffed love letter to prog-rock operas like Tommy and The Wall." Brendon Small won a BMI TV Music Award for the score.

For a decade, The Doomstar Requiem was seen as the de facto series finale, as Adult Swim canceled further episodes. However, fan demand and a Kickstarter campaign eventually led to the 2023 movie Metalocalypse: Army of the Doomstar, which directly follows the opera and concludes the story.
